FAR Group is currently seeking Special Needs Coordinator to support the Exceptional Family Member Program-Medical (EFMP-M).

Duties and Responsibilities:

· The Special Needs Coordinator shall perform a full spectrum of services as an Exceptional Family Member Program-Medical (EFMP-M) Manager. The SNC oversees and manages the installation EFMP-M IAW DoD and Air Force policy, and any subsequent implementing guidance.

· The SNC uses a multi- disciplinary and collaborative approach with other key service providers, such as installation youth and childcare facilities, officer and enlisted spouses groups, and DODEA schools to ensure effective outreach and identification of special needs. Coordinates with AFRC and AFPC as required for special need families.

· Conducts assessments of family members of RegAF sponsors to determine if special medical and/or education conditions exist that require EFMP enrollment.

· Ensures all RegAF sponsors known to the MTF as having family members with special needs according to DoD criteria are identified to the AFPC (via the local MPS and/or CSS, where applicable) for issuance of an ALC “Q”, regardless of the location of family members or the source of family health care.

· Ensures all Army, Navy, and Marine Corps sponsors of family members with special needs are referred to their Services’ EFMP POCs for enrollment and relocation support IAW Service-specific guidance.

· Ensures all RegAF sponsors known to the AFPC, and local MPS and/or CSS where applicable, as having family members with special needs are identified in AF medical special needs data management systems (MyVector).

· Ensures every RegAF sponsor with one or more family members with special needs that are assigned to the installation has a Family Profile established in MyVector, along with an SN file maintained within the member’s EFMP Enrollment account.

· Coordinates, with AFPC (and the local MPS and/or CSS, where applicable) all assignment actions for all Q-coded AF sponsors assigned to the installation, or whose personnel functions are handled at the installation, regardless of location of family members.

· Complies with requirements for data security, maintenance, collection, and reporting IAW AFPD 40-7 and any subsequent AFPC, MAJCOM, or MTF implementing guidance.

· The SNC Provides AFPC/EAP the MTF/CC-signed appointment letters for the SNC, SNT, and alternates for access to password-protected data systems.

· Establishes procedures for the creation, maintenance, secure storage, transfer and retirement of SN files and FMTS files IAW AF policy, EFMP-M guidance, and MTF guidance for the protection of PII and PHI.

· Ensures that sponsors are provided all necessary documents for enrollment in TRICARE, Extended Health Care Options (ECHO). Where the SNC may not release protected health information to the sponsor, the SNC provides the needed documentation to the identified patient of majority age or forwards the information directly to the TRICARE Managed Care Support Contractor or other authorized Health Affairs agent upon request.

· Coordinates with MTF TRICARE representatives to provide information to beneficiaries about community, state, and national resources specific to the special needs population.

· Ensures Q-coded sponsors assigned to the installation are contacted annually to update their EFMP status, and provide any updates needed in contact information, family composition, or family member conditions; verification of need for continued enrollment as well as pending PCS should also be discussed.

· The SNC maintains a cooperative working relationship with AFPC, installation MPS, CSS, and all other associated offices for the following duties:

· Completion of the TSQ for outbound personnel, authorization letters for initiation and deletion of Q-codes, prompt initiation of the FMTS process at notification of pending OCONUS assignment (if Q[1]coded, include CONUS), or eight (8) months prior to follow-on after an unaccompanied assignment, support to EFMP reassignment/deferments, and regular delivery of the base Q-code roster from the local MPS/CSS to the SNC.

· The SNC shall coordinate with DODEA to obtain IEPs monthly, update log and update or add Q Code to AD family members.

· Candidate shall be skillful and tactful in communicating with people who may be physically or mentally ill, uncooperative, fearful, emotionally distraught, and potentially agitated.

· Ensures the appropriate documentation of EFMP-M workload as a military-specific mission in consultation with the MTF Medical Expense and Performance Reporting System (MEPRS) monitor using MEPRS code FAZN for all EFMP-M associated workload.

· Will actively support the integration of EFMP-M, EFMP-FS, and EFMP-A services at the installation.

· Participates with EFMP-FS in Newcomer’s Orientation and relocation briefings to ensure coordinated transitions for EFMP families.

· Shall collaborate with and participate in the Community Action Team (CAT) to address EFMP needs at the installation level and any other interagency forums that provide family medical information and referrals to base and civilian agencies.

· The SNC shall be responsible for the processing and tracking of EFMP assignments through the My Vector application and if applicable the legacy Q-Base system.

· Completes medical record documentation and coding and designated tracking logs and data reporting as required by local MTF/AF/DoD instructions, policies and guidance.

· Completes all required electronic medical record training, MTF-specific orientation and SNC training programs.

Skills:

· Knowledge. Must have the knowledge and skills to effectively apply SNC functions.

· Assessment: The SNC must have the ability to identify patients and families with special medical and educational needs.

· Planning: Collaboration with the patient, family/caregiver, primary provider, and other members of the health care team for developing an effective plan of care is expected.

· Facilitation: Care coordination and communication among all involved parties is key to provision of care for individuals with special medical needs.

· Advocacy: Support for the patient and family/caregivers to ensure proper education of the patient and family and appropriate, timely care coordination is received.

· Familiarity with clinical conditions that require specialized care and demonstrated skills in interviewing families for the purposes of identifying special needs and for coordinating needed care.

· Candidate shall be able to read, write, and speak English well enough to effectively communicate with all parties and other health care providers.

· The Candidate shall possess sufficient initiative, interpersonal relationship skills and social sensitivity such that he/she can relate constructively to a variety of patients from diverse backgrounds.

Work Schedule:

8-hour workday Monday through Friday from 07:30- 16:30.

Qualifications:

· Life support training. At a minimum, the SNC must maintain current certification in either the American Heart Association Basic Life Support (BLS) (Course C) or the American Red Cross BLS Course.

· The HCPs shall maintain all license and certification requirements and meet all continuing education requirements of the U.S. jurisdiction in which they are licensed to practice.

· Education: Must possess a baccalaureate degree in nursing (BSN) accredited by a national nursing accrediting agency recognized by the United States Department of Education. Foreign trained nurses are required to have an active certification by the Commission on Graduates of Foreign Nursing Schools (CGFNS).

· Licensure/Certification/Registration: Must possess a valid, unrestricted license to practice as a Registered Nurse (RN) from one of the 50 United States, and/or the District of Columbia, the Commonwealth of Puerto Rico, or territories of the United States. The contractor must be in good standing, and under no restrictions, or investigations with the state in which a license is held or has been held, or the national licensure board, within the past 10 years.

· Experience: A resume that reflects at least 2 years of full-time experience in care coordination, discharge planning or case management in medical (military or civilian) setting within the last 5 years.

· English for Non Native Speakers: Contract personnel shall have proficiency in English both orally and written. English proficiency shall be verified through submittal of a TOEIC score greater than or equal to 850.
